My most memorable death was when i played my Sin in HA.
Dark Chambers. Our guild versus either IWAY or some other team with a trapper. I played Aura of Displacement sin and was normally there to kill the enemy priest solo while the rest of our team double backed to save ours. Yeah well that time i was just using AoD to get to the priest and literally exploded cause the enemy team trapped the hell out of the priest. Me using frenzy wasnt exactly helping either

most memoriable death:
I was running BiP/BR in random arena w/ 1 hp (hey, i have my reasons )
The opponents included 3 rangers and 1 monk. Since the rangers got so tired of me dodging their arrows, one of them put up the spirit that increases your max hp for every enchant you had. Basically my health went from 1hp -> 70hp. I got killed in two shots xD
responsible for other ppl's death:
I was playing monk in HM gate of madness when my friend started pm-ing. I got distracted with talking that by the time i finished, everyone had left the team (i assumed that they were all dead)

My first time tanking in UW, I used Hundred Blades on 3 closely grouped Smite Crawlers. All of the smites had Shield of Judgment up. An instant ~600 holy damage.

Getting killed by 1 smite was my most embarassing... hmm i think its safe here ill go get some tea, come back have 10health and the smite finishes me off...
Worst death: unloking a chest in fow and seeing a req8 chaos axe drop only to be killed with noone to res :'( i never went back to FoW since. The memories plague me XD

when i was 1k xp off from the surviver title i was helping my friend out (he was new to GW) by doing the Fort Ranik mission, i went down to the 3 flame things to pick up items and then he fired the catapult, 4 times, wondering what it did after seeing me use it, gah! died 3 times! no more surviver title.

My personal favorite was when I went vanquishing with my buddy (he was bonding) and we ran into a rit boss. Over vent I heard him say "holy #%Q# hes huge". He ran up for a closer look at this boss and got hit by spirit burn. Instant death. Boss then proceeded to kill us off one by one.

My most memorable, is also my most ironic.
I was on my Ritualist, who is a level 1 survivor, and I was planning on going for at least level 2. Because of this, I was capping elites for their exp boosts.
I was fighting Ssyn Coiled Grasp for his elite, when I was hit by his [wiki]Ancestors' Rage[/wiki] which brought me down to about 20 health. I was then killed by the Warrior standing next to me.
Now this was ironic because, my Ritualist was wielding Ssyn's Staff. I got the staff almost half a year earlier when I excessively farmed Ssyn for his staff.

My favorite Death: In random arenas i was an SS(necro) and as soon as we entered (without a monk) i hear
Leet Pwner PvP(fake but i remember his name): OMFG WTF U NOOBS NO MNOK!!!(yes he misspelled it)

The other team consisted of four assassins who procided to taunt me
the grouped around me and dances
i threw up reckless haste and echoed SS and got two of em
all of the sudden the attacked i saw about 50 or so 37's and 3 of the four dropped with 1 standing who proceded to /rank my corpse but then
our tank came back and killed him in 1 hit
We have been friends ever since.
My most hated death
FoW opening the chest to see a chaos axe r8 on the floor running to it getting close...closer...closer stoping right in front of it and then .....EER7
i came back to find one enemy monk over my corpse and my guildies laughing their asses off

This has happened to me twice now, once in RA and once in TA with guildies-
I sometimes run Death Pact Signet on my Rit, because if your team is competent enough, a fast and renewable rez can kick major ass. Of course, it can backfire. I try to be the last to use my rez because of its downside. Sometimes I have to rez multiple people. Well, twice so far, someone I rezzed died and killed me right along with them. One of our surviving team members rezzed me, then the other person I rezzed died just seconds thereafter, killing me again. So there I was taking a dirt nap with 2 deaths in a span of seconds, and 0% DP.

When i was doing 55/SS UW. I died and got resed, and then i equipped the -20 offhand.... 13 HP!!!
And then an Aatxe severed my artery and then a Dying Nightmare stripped my enchants. And then my nice little pink health bar changed to dark red in 2 seconds....
Or, when i was capping Avatar of Lyssa as lvl 17. I got killed by the Acolyte, then my henchies res sigged me while the acolyte was next to me. "whee full health!" and then the acolyte hit me with twin moon sweep, dead in 1 hit. So one of the henchies ressed me again. Twin moon sweep again, i died in 1 hit. happened at least 5 times.
but i managed to kill it in the end

When I was trying to map the bonus mission area of the Dunes of Despair mission. I had teleported myself to the outside using Necrotic Traversal to be able to map without the timer started.
After mapping quite a bit I got killed by those pesky sand scarab monsters. After some time I suddenly saw some green dots appear on the radar. My henchies were coming to rezz me!
With a full group I could easily map all area end even do the bonus mission. Thank god for this glitch!

Playing one of the Fire Island coops where you have to choose whether to attack the main gate or take the side entrance into the Mursaat fortress, I remember I was playing as my Warrior and the team had gotten pretty much wiped out. There was me and there was one monk left running for our lives and the Mursaats had aggro'd the monk, I turned around, body blocked the mursaat and valiantly gave my life breaking the aggro on the monk.
Then the monks got error007'd and this was before the update when you could come back into the game..
Which kinda hurt coz I could have rez'd the Ritualist if I hadnt sacrificed myself to save the monk.
Oh well c'est la vie.

My most memorable deaths were helping a friend that was seeking for survivor title.
Me and my henchies and heros were his portable bombs, He putted death nova in all us and after some dodyblocking on the boss we begun to use BiP. It was kinda funny

It's not my character but definately the most memorable deaths of a party member (he's my guildie in this case). I was with my guildie killing Varesh. Guildie Dervish was killed by Call of Sacrifice (Hex spell: For 20 seconds or until target is at less than 20% Health, target foe loses 20 Health each second.)
The Hex stayed with the character even after death and no matter how many times tried to rez him, as soon as he got up, he is dead again before even being able to stand up straight!
Not knowing what was going on at first (were too focused on killing Varesh), the monks were rebirthing him all over the place - so he was literately flying/teleing all over the screen!! And only after realising the dying sound of the character can be heard repeatedly (the "Eurrrr" sound before the character collapsed onto the floor), and him spamming "Please don't raise my DP & deaths anymore!!"... we stopped trying to rez him and managed to kill Varesh and finished the mission.
P.S Anet, there's a bug there!!
